Questions and Referrals

Direct inquiries begin with outreach concerning program offerings, which can be made through email, phone calls, or the website. Referrals are welcomed from family members, healthcare providers, self-referrals, or alternative methods.


Subsequently, a virtual screening assessment is performed, Home Visits can be conducted if preferred. An in-home evaluation is core procedure to determine the required level of support needed. This process also involves the development of a care plan, detailing service costs if applicable, along with the establishment of a service agreement.

Service Delivery

The Active Support period commences with tailored in-home delivery of chosen services, including benefit advocacy, minor home safety repairs, housing matching, technology assistance, and fraud prevention, among other services. Duration of support services varies based on needs; then followed by stabilization assessment and case closure which lasts as long as is required to satisfy the needs of our clients and caregivers. For re-engagement (if necessary), clients have the option to reactivate their cases or request additional services.
